VLT Reopening Approval Request

To request a review to have your VLTs enabled:

1. Take photos that show each VLT including any installed physical barriers and a wide-angle picture of the room where your VLTS are located.
2. Note the position numbers of the terminals you would like to have approved. The position numbers are on your end of day reports.
3. Submit the photos, business name, retailer identification number and position numbers in the form below

NOTE: If your county is still in “Extreme Risk”, your VLTs will not be enabled, even if approved, until the county returns to a lower risk classification.
